Handicapped Placards
The state of Alabama
makes provisions for those with severe handicaps and
physical limitations to obtain "Handicapped Placards" so
that they may legally occupy parking spaces reserved for
patients with either temporary or permanent handicaps.
Please be aware that there are specific criteria (you will see these listed on the form) which must be met in order to certify the necessity of this placard, and it is our policy to adhere strictly to these rules.

to our office for certification next visit, and we will
be happy to help you with this.

room for clinical judgment, we will not "bend" the rules
